  • Applicants register online via the Goa government services portal (goaonline.gov.in).
  • They must provide essential details about the family and upload the necessary documents, including residence proof covering at least 5 continuous years.
  • Fees are charged as Rs. 200 for families with three or fewer members and Rs. 300 for families with four or more members, with a 50% concession for SC/ST, OBC non-creamy layer, and differently-abled persons.
  • After submission, applications are verified by the concerned offices. Approved applicants receive a DDSSY health card which they must collect within 30 days from the selected office.
  • Panchayats help residents by providing assistance with document verification, facilitating online registration, and guiding applicants, especially in rural and semi-urban areas.
